Exploding Head Syndrome: That Bang In Yer Skull

Nighttime noises are one thing. Your mate upstairs with a dodgy telly is another. But some folk wake up or nod off to a sound so dramatic they think a lorry's hit the gable. Thats exploding head syndrome, and no, youre not actually being bombed from the inside.

It is a real parasomnia. People report a single, massive-sounding bang, crash, gunshot or flash of light at sleep onset or on waking. There is no pain from the noise itself, just panic, heart racing and that proper startled feeling like youve been mugged by silence. Some even get a flash behind the eyes for a second.

Doctors do not have a neat answer for why it happens. The brain is a grumpy machine at sleep transitions, and one idea is parts of the brainstem or temporal lobe misfire when the mind is switching from wake to sleep or back again. Stress, being knackered, bad sleep patterns and odd shift hours make it likelier. Basically, if youve been treating sleep like a small suggestion, your brain might throw a tantrum.

People worry it is a stroke, an aneurysm or epilepsy. Fair enough. It can look scary. But exploding head syndrome is usually harmless. Rarely it is so frequent or upsetting that someone ends up proper anxious about sleeping. Then they should see a doctor or a sleep specialist so they can check for other causes and talk strategy.

Treatment is mostly reassurance and sorting your sleep. Regular bedtimes, less late noise and stress-sorting tend to calm it. There are a few case reports where some medications, like certain antidepressants or anticonvulsants, helped, but those are for odd stubborn cases, not the usual bang-and-forget.

I once woke with one of these and thought someone'd set off fireworks inside my head. Stood there rubbing my face like id been in a cartoon. You feel stupid later, but at least you know the building is still standing. Sleep properly, sort the stress, and your brain will stop auditioning for a war film in the middle of the night.
